In Pennsylvania, students identified as needing special education services require a preapproval letter by a qualifed person  to be provided with the affidavit/unsworn declaration and objectives.  

How do I receive a preapproval letter?

1. Complete and submit the form below.

2. The cost for a preapproval letter is $25.  (If I complete an evaluation for your student and you need a preapproval letter for the following school year, there will be no additional charge.)

3. To receive a preapproval letter, you must provide the most current documentation (evaluation report or re-evaluation report and IEP) and educational objectives. 

4. Upon submission of this form, you will be emailed at the email address you provided and given directions for sending needed information and payment.  Once all information is provided and payment received/cleared, you will receive your signed preapproval letter within a week via an emailed PDF. 

5. Payment forms include:  PayPal, Venmo, Zelle, Apple Cash, Cash App, or mailed check.
